AGREEMENT TO THE OFFICIAL RULES: By participating in the Sweepstakes, all entrants fully and unconditionally agree to and accept these Official Rules and the Sponsor’s decisions, which are final and binding in all matters related to the Sweepstakes. Winning the prizes (described below) is contingent upon fulfilling all requirements set forth herein.
SWEEPSTAKES PERIOD: The Ideal Image Membership Madness Sweepstakes (“Sweepstakes”) begins at 8:00 a.m. EST on March 4, 2021, and ends at 11:59 p.m. PDT on March 31, 2021 (“Sweepstakes Period”). Sponsor’s computer is the official timekeeping device for the Sweepstakes.
SPONSOR: Ideal Image Group Inc., 1 North Dale Mabry Hwy, Tampa, FL 33609 (“Sponsor”). Sweepstakes is not sponsored by or affiliated with Curiouser Products Inc. d/b/a MIRROR.
ELIGIBILITY. The Sweepstakes is open only to individuals who, at the time of entry, are 18 years or older and legal residents of the 48 contiguous United States or District of Columbia, who have reached the age of majority in their province of residence. Employees, directors and officers and their immediate families (husbands, wives, brothers, sisters, parents and children and their spouses) and household members (whether or not related) of Sponsor and its parents, subsidiaries and affiliates, and companies involved in the implementation and execution of the Sweepstakes or otherwise associated with the Sweepstakes (collectively the “Sweepstakes Entities”) are not eligible.
PRIZES AND THE VALUE OF THE PRIZES. Two (2) Grand Prizes: Each Grand Prize consists of one (1) Mirror home gym and a one (1) year Mirror home gym class subscription. The approximate retail value (ARV) of each Grand Prize is US$1,963. ARV of all prizes is US$3,926. Any difference between the stated ARV and the actual value, if any, will not be awarded. Prize is awarded “as is” with no warranty or guarantee, either express or implied by Sponsor. All prize details are in Sponsor’s sole discretion. Winner is solely responsible for all costs and expenses (including taxes) not specifically described herein. Prize will be delivered to winner at the address provided by winner in the entry form approximately 6-8 weeks after winner confirmation.
GENERAL PRIZE RESTRICTIONS. The prize is subject to availability and is subject to change. The prize may not be transferred, assigned, changed or redeemed for cash, except at the sole discretion of Sponsor. The Sponsor reserves the right to substitute the prize in whole or in part with other prizes of comparable or greater value if the intended prize is not available for any reason as determined by the Sponsor in its sole discretion. US resident winners will be required to provide their Social Security numbers and appropriate tax documentation for tax reporting purposes. Sponsor will file where required an IRS Form 1099 with the Internal Revenue Service for the fair market value of any prize.
HOW TO ENTER: There are two ways to enter:
- Purchase Method: Visit a participating Sponsor location (or participate in a virtual consultation) and purchase a Lifetime Guarantee Membership (“Membership”). Upon completion of the Membership purchase, the participant will receive one entry.
- Free, Non-Purchase Method: Mail a 3×5 postcard with the statement “I would like to enter the Membership Madness Contest for a chance to win a Mirror Home Workout + 1 Year of Mirror Home Workout Membership” as well as your name, address (including zip or postal code), email address, and phone number to:

Mail-in entries must be postmarked by March 31, 2021 and must be received by April 7, 2021. Limit one (1) post card per outer envelope.
All entries become the property of the Sponsor and will not be acknowledged or returned. No entries will be acknowledged or returned. Sponsor assumes no responsibility for lost, mutilated, late or misdirected entries. Limit one (1) entry per person.
Any non-complying entry will subject the entrant to disqualification in Sponsor’s sole discretion. Sponsor is not responsible for entries that are incomplete or inaccurate whether caused by any technical or human error, which may occur in the processing of entries. Sponsor is not responsible for computer system, phone line, hardware, software or program malfunctions, or other errors, failures or delays in computer transmissions or network connections that are human or technical in nature. By submitting an entry, you agree that your submission is gratuitous and made without restriction, and will not place Sponsor under any obligation.
TIMING OF DRAWINGS AND NOTIFICATION TO WINNERS. Two Grand Prize winners will be selected via a random drawing on or about April 9, 2021 consisting of all entries received by Sponsor. The provisional winners will be notified by Sponsor using the information provided in the entry form. If a provisional winner cannot be contacted or fails to return any requested documentation within ten (10) days of notification, an alternate winner may be selected by Sponsor from all remaining eligible entries in a second-chance drawing of all eligible entrants. Selection of winners will be conducted by Sponsor, whose decisions are final.
As a condition of receiving a prize, the provisional winner may be required to complete, sign and have notarized an affidavit of eligibility, a liability release or similar documents as outlined by Sponsor and except where prohibited by law, a publicity release, which must be returned within the date set forth in the winner notification. Failure of the provisional winner to timely sign and return any required documents or the return of any prize or prize notification as undeliverable, or failure of the provisional winner to claim the prize or to comply with any term or condition of these Official Rules, may result in disqualification, the forfeiture of his or her interest in the prize, and the award of the prize to a substitute provisional winner.
A PARTICIPANT IS NOT A WINNER OF A PRIZE UNLESS AND UNTIL SPONSOR HAS COMPLETED ITS VERIFICATION OF PARTICIPANT’S ELIGIBILITY AND COMPLIANCE WITH THESE OFFICIAL RULES.
ODDS OF WINNING. Odds of winning will depend upon the number of eligible entries received.
WINNERS’ LIST. For the names of the winner, please send a self-addressed and stamped (for US residents) envelope to Ideal Image Group Inc., Attention: Tracy Knapp or Kendra Lloyd, Membership Madness Entry Winner List, 1 N Dale Mabry Hwy Suite 1200, Tampa, FL 33609 Requests for winners’ names must be received by June 15, 2021.
GENERAL CONDITIONS AND RELEASE. By participating, each entrant (a) agrees to release Sponsor, and each of its respective parents, affiliated companies, subsidiaries, licensees, distributors, dealers, retailers, printers, representatives and advertising and promotion agencies, and any and all other companies associated with the Sweepstakes, and all of their respective officers, directors, employees, agents and representatives from any and all liability for any claims, costs, injuries, losses or damages of any kind that may occur, directly or indirectly, in whole or in part, by their participation in this Sweepstakes, including the unauthorized or illegal access to personally identifiable or sensitive information or the acceptance, possession, use, or misuse of the prize except where prohibited by law; (b) consents to the use by Sponsor and each of their respective designees of the winner’s name, hometown, photograph, voice, or other likenesses and prize information for promotion, advertising and marketing purposes in any media now known or hereafter devised, throughout the world, in perpetuity, without additional compensation or consideration, notification or permission, unless prohibited by law, and upon request will provide written confirmation of such consent; and (c) acknowledges that none of the Sweepstakes Entities or anyone else has either made, or is in any manner responsible or liable for, any warranty, representation or guarantee, expressed or implied, in fact or in law, relative to any prize including, but not limited to, its quality or availability. Neither any of the Sweepstakes Entities nor any service providers are responsible for incorrect or inaccurate transcription of entry information, or for any human or other error, technical malfunctions, lost or delayed data or voice transmission, omission, interruption, deletion, defect, line failures of any telephone network, computer equipment, software, inability to access any online service or website or to complete a telephone call or facsimile transaction, or any other error or malfunction, or late, lost or misdirected mail, or any injury or damage to an entrant’s or any other person’s computer related to or resulting from participation in this Sweepstakes.
CAUTION: ANY ATTEMPT BY AN ENTRANT OR ANY OTHER INDIVIDUAL TO DAMAGE ANY WEBSITE, TAMPER WITH THE ENTRY PROCESS, OR OTHERWISE UNDERMINE THE CONTENT OR LEGITIMATE OPERATION OF THE SWEEPSTAKES MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR WILL DISQUALIFY ANY SUCH ENTRANT, AND SPONSOR AND THE APPLICABLE ENTITIES RESERVE THE RIGHT TO SEEK DAMAGES AND OTHER REMEDIES (INCLUDING ATTORNEY FEES) FROM ANY SUCH ENTRANT OR INDIVIDUAL(S) TO THE FULLEST EXTENT PERMITTED BY LAW.
No failure to enforce any term of these Official Rules shall constitute a waiver of that provision. If for any reason the Sweepstakes (or any portion thereof) is not capable of being executed as planned, or in the case of infection by computer virus, bugs, tampering, unauthorized intervention, fraud, action of entrants, technical failures or any other causes that in the opinion of Sponsor or Sponsor, corrupt or affect the administration, security, fairness, integrity, or proper conduct of this Sweepstakes (or any portion thereof), Sponsor reserves the right in their sole discretion to disqualify any suspect entry or entrant and to cancel, terminate, modify or suspend the Sweepstakes (or any portion thereof). Entries not complying with all rules are subject to disqualification.
DISPUTE RESOLUTION: Except where prohibited, by participating Sweepstakes entrants agree that: All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of participant(s) and Sponsor and its agents shall be governed by and construed exclusively in accordance with the laws of the State of Florida without giving effect to any principles of conflicts of law of any jurisdiction. Entrant agrees that any action at law or in equity arising out of or relating to this Sweepstakes, or awarding of the prizes, shall be filed only in the state or federal courts located in Hillsborough County, Florida and entrant hereby consents and submits to the personal jurisdiction of such courts for the purposes of litigating any such action. Except where prohibited, by participating in this Sweepstakes, each entrant agrees that: (a) any and all disputes, claims, and causes of action arising out of or connected with this Sweepstakes, or awarding of the prizes, shall be resolved individually, without resort to any form of class action; and (b) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with participating in this Sweepstakes but in no event attorneys’ fees; and (c) under no circumstances will any participant be permitted to obtain awards for and hereby waives all rights to claim punitive, incidental and consequential damages and any other damages, other than for actual out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased. Some jurisdictions do not allow the limitations or exclusion of liability for incidental or consequential damages, so the above may not apply to you.
